Job Title:   Family Navigator II

Department:    Family Support Services

 

Supervisor:    Family Development Coordinator

Employee Status:    Part-Time: 18 hours/week (as funding permits)

 

Description:    The Family Navigator works in conjunction with the Family Advocates and Data Manager to ensure that families eligible for family pathways services are provided professional and culturally sensitive supports throughout their association with the program. 

 

Essential Duties and Responsibilities:

Work in conjunction with the Family Development staff: 

·        Screen families for interest in Family Development services

•             Assist families to complete required intake forms

•             Review Common Screening Questionnaire of intake form to provide appropriate referrals to in-house Family Advocates

•             Data entry into FRCAforce

•             Provide emergency assistance referrals (housing, utilities, medical, food bank, etc)

•             Answering general questions about the Family Support programs and the La Plata Family Centers Coalition’s services and refer families to all applicable programs/resources with LPFCC

•             Assign families to the appropriate Family Advocate

•             Scheduling appointments with families

•             Making referrals to community resources as needed

•             Data entry/tracking as needed for grant reporting in conjunction with the Data Manager

•             Attend all program, all-staff, and appropriate community partnership meetings

•             Attend CFSA 2.0, MI, Family Pathways, Protective Factors, EOC trainings and any other trainings necessary to facilitate family support services

•             Assist with other tasks and duties as deemed necessary by the Deputy Director

 

Qualifications:

             Professional administrative skills

             Supportive and positive attitude

             Excellent communication, both verbal and written

             Strong interpersonal and organizational skills

             Computer literacy (Microsoft Office, Publisher, Windows, Internet/Email, etc.)

             Must have a valid Colorado driver’s license

             Must be willing to travel locally and occasionally regionally to partner counties (Archuleta & San Juan)

             Must be able to pass a criminal background check
