The La Plata Family Centers Coalition is a community-based organization that delivers high-quality human services in rural Southwest Colorado. For over 35 years, The La Plata Family Centers Coalition has listened and responded to the current social, emotional, educational, and economic needs of the diverse families in our community.

The Colorado Community Response program is designed to help prevent child maltreatment, by strengthening families economic security, improving their ability to cope with stress, and helping families measure an increase in positive progres

Family Support
Services​

Family Support Services pairs caring family advocates with families to achieve self-sufficiency by providing the following: goal setting, addressing immediate needs through resource sharing and referral networks, financial coaching, and parenting education.

Parents As
Teachers

The Parents as Teachers  home visitation program is offered to families with young children and includes parenting education through Love and Logic workshops as well as free hearing, vision, and developmental screenings.

Durango, CO – La Colorado Health Foundation ha otorgado un subsidio de $478 000 a LPFCC para establecer el proyecto “Together We Grow/Juntos Crecemos”. Los miembros de la comunidad encabezarán esta iniciativa para utilizar, arreglar y ampliar la infraestructura existente en la propiedad de Sunnyside Farms ubicada en County Road 215, al sur de Durango. La meta de la iniciativa es la de promover la salud en nuestra comunidad, cultivando un suministro de alimentos local y sostenible, mejorando la estabilidad económica, y fomentando el orgullo por la herencia cultural de múltiples generaciones.

Together We Grow – Press Release

Durango, CO- The Colorado Health Foundation has awarded $478,000 in grant funding to LPFCC to establish the “Together We Grow/Juntos Crecemos” project. Community members will lead this initiative to utilize, repair and expand on existing infrastructure on the Sunnyside Farms property located off County Road 215 south of Durango. The goal of the initiative is to promote health in our community by growing a local, sustainable food supply, enhancing economic stability, and encouraging pride in multigenerational cultural heritage.
